Stuart Freeborn

Stuart Freeborn (born 1914 September 5, in London, England) is a British motion picture make-up artist, perhaps best known for his work on the original Star Wars trilogy.
He created the make-up for all of the characters, including Yoda, whom he supposedly based on his own face and that of Albert Einstein. Freeborn was also the make-up artist on 1968's 2001: A Space Odyssey, where he created the humans/apes for the "Dawn of Man" sequence.
In The Phantom Menace DVD in one of the web documentries, he was awarded a statue by the team at Lucasfilm. [1]
